As stated in the parent agreement report any changes in employment, job seeking, incapacitation, or training status; family size (including marriage, birth of a child, mother or father of the child coming into or leaving the household), hours of care needed, income, address, or phone number to my Subsidy Counselor within 5 calendar days.  Failure to report these changes is grounds for termination from the program.
