Healthy and Active Before 5 (HAB45) is a collaborative initiative in Contra Costa County. Their mission is to ensure that all families, especially those of color with limited resources, have opportunities to thrive. They advance health and racial equity, policy change, and coordinated collaborative strategies to improve the health and well-being of young children and their families.
